EEOC Charge Form

An EEOC Charge Form is a document used to file a formal complaint of employment discrimination with the U.S. Equal Employment Opportunity Commission (EEOC). The form requires the complainant to provide information about themselves, their employer, and the alleged discriminatory acts, including the basis for the discrimination (such as race, sex, age, or disability), the dates and nature of the discriminatory acts, and any witnesses or supporting evidence. The form must be signed and verified by the complainant, and must be filed within 180 days of the alleged discriminatory act (or 300 days in some states with their own anti-discrimination laws). The EEOC Charge Form is the first step in the EEOC’s investigation and enforcement process, and is a necessary prerequisite for filing a lawsuit under federal anti-discrimination laws. It is an important tool for individuals who believe they have been subjected to unlawful discrimination in the workplace, and who wish to seek legal redress and remedies.
