Evaluation Report

An Evaluation Report is a written document that summarizes the results of a comprehensive assessment of a student’s educational needs and eligibility for special education services. The evaluation may include a variety of assessments, such as cognitive, academic, behavioral, and social-emotional measures, as well as input from parents, teachers, and other professionals. The Evaluation Report typically includes a description of the student’s strengths and weaknesses, as well as recommendations for educational programming and support services. The Evaluation Report is used to determine whether a student meets the criteria for a specific disability category under the Individuals with Disabilities Education Act (IDEA), and is a key component of the special education referral and eligibility process.
