Behavioral Data Collection Sheet

A Behavioral Data Collection Sheet is a tool used by teachers, paraprofessionals, or other school staff to systematically observe and record a student’s behavior over a specified period of time. The sheet typically includes a list of specific behaviors that are being targeted for intervention or monitoring, along with a system for recording the frequency, duration, or intensity of each behavior. The sheet may also include information about the antecedents (i.e., triggers or events that occur before the behavior) and consequences (i.e., reactions or outcomes that occur after the behavior) associated with each instance of the behavior. Behavioral Data Collection Sheets are often used as part of a functional behavior assessment (FBA) or a behavior intervention plan (BIP) to help identify patterns or trends in a student’s behavior and to evaluate the effectiveness of specific interventions or strategies.
