Record of Decision
A Record of Decision is a written document that summarizes the decisions made by an IEP team regarding a student’s special education program and services. The Record of Decision typically includes a description of the student’s present levels of performance, the goals and objectives that have been set for the student, the services and supports that will be provided to help the student achieve those goals, and any other relevant information or considerations. The Record of Decision is developed at the end of an IEP meeting, and is signed by all members of the IEP team, including the student’s parents or guardians. The Record of Decision serves as a formal record of the IEP team’s decisions and a guide for implementing the student’s special education program. It is an important tool for ensuring that all members of the IEP team are in agreement about the student’s needs and the appropriate services and supports to address those needs.
