A Service Delivery Plan is a written document that outlines the specific special education and related services that a student with a disability will receive, as well as the frequency, duration, and location of those services. The Service Delivery Plan is typically developed as part of the student’s IEP, and is based on the student’s individual needs and goals. The plan may include services such as specialized instruction, speech and language therapy, occupational therapy, physical therapy, counseling, and assistive technology, as well as accommodations and modifications to the general education curriculum. The Service Delivery Plan is developed with input from the student, their parents, teachers, and other relevant professionals, and is reviewed and revised annually. The Service Delivery Plan is an important tool for ensuring that students with disabilities receive the individualized supports and services they need to make progress in their education and achieve their full potential.
