A Claim Documentation Checklist is a list of the various documents and information that a policyholder or claimant may need to provide to an insurance company in order to support a claim. The checklist may include items such as proof of loss forms, police reports, medical records, repair estimates, and receipts, among others. Claim documentation checklists are often provided by the insurer at the outset of the claims process to help the policyholder understand their obligations and expedite the resolution of the claim.
