A Request for Provisional Ballot is a form filled out by a voter who believes they are entitled to vote but whose eligibility cannot be immediately confirmed at the polling place. Reasons for requesting a provisional ballot may include the voter’s name not appearing on the registration list, lack of proper identification, or a discrepancy in the voter’s information. The request form captures the voter’s personal details and the reason for the provisional ballot. The voter is then allowed to cast a provisional ballot, which is kept separate from regular ballots until the voter’s eligibility can be verified. If the voter is determined to be eligible, their provisional ballot is counted. Provisional ballots serve as a safeguard to ensure that no eligible voter is turned away from the polls.
