Form N-648 is a medical certification filed by naturalization applicants who are seeking an exception to the English language and/or civics testing requirements based on a physical or developmental disability or mental impairment. The form must be completed by a licensed medical professional who certifies that the applicant’s condition prevents them from meeting the testing requirements and that the condition is expected to last at least 12 months. Form N-648 is filed with Form N-400 (Application for Naturalization) and is reviewed by USCIS to determine whether the applicant qualifies for the disability exception.
