Proof of Sponsor's U.S. Citizenship or Lawful Permanent Resident Status

If the respondent is being sponsored for adjustment of status by a U.S. citizen or lawful permanent resident, proof of the sponsor’s status must be submitted. This may include a copy of the sponsor’s U.S. birth certificate, naturalization certificate, or Green Card.
