Form I-90 (Application to Replace Permanent Resident Card)

Form I-90 is an application filed by lawful permanent residents whose Green Card has been lost, stolen, damaged, or needs to be replaced for other reasons, such as a change of name or expiration of the previous card. The form requires the applicant to provide personal information, the reason for the replacement, and any required supporting documents. In some cases, the applicant may also need to attend a biometrics appointment to provide fingerprints, photographs, and/or a signature.
